New patients coming for their annual physical only need to print out the New Patient Form and the Annual Physical Questionnaire. If you have specific questions about which forms are needed for your visit please call our office for details.

Transfer your medical records with us to another provider or request records for your personal health file. You may print the form and send it to our office via email at myhealthcare@lakeside-family-physicians.com or fax to 425-395-0245. Please allow up to two weeks for processing before calling us on the status of your request.

It is our policy to provide a continuity of care record, any imaging, lab work and recent chart notes pertinent to the patient's care when “All Records” are requested. If you believe you need more than this outline information please specify in your request.  

Patient's requesting "All Records" for their personal files are subject to processing fees as laid out by the Washington State Department of Health which have been recently updated as of June 9th 2021. The rates are the following: Pages 1-30 are $1.24 per page, pages 31 and up are $0.94 per page.

However, there is no charge associated if you are interested in your continuity of care record, any imaging, lab work and recent chart notes pertinent to your healthcare. If this is the case, please select "Abstract/Summary" on our form for requested records.

Requesting a smaller amount of information will get your request placed to the top of the list and can expedite your processing time.

Lakeside Family Physicians accepts Visa, MasterCard, American Express, and Discover, debit cards, health savings account cards, personal checks, and cash. Copays and cash pay office visits are due at the time of service.

As a courtesy to our patients, we will submit your medical claims for you to your insurance. It is your responsibility to know before your appointment if you are eligible for services. If not you could be subject to cover the entire cost of the visit rather than the discounted cash pay office visit charge. Payments are due 30 days after the receipt of your statement. Failure to send payment may results in a $20 billing late fee. Services that are 120 days past due will be sent to the Physicians and Dentists Bureau for collections. 

If you are experiencing financial difficulty, please fill out the Financial Hardship Application here and send it to us either via email or bring in a hard copy to the clinic. You may be qualified for discounted cash pay rates. 

Laboratory services are outsourced to LabCorp and Northwest Physicians Laboratories. Most patients will have some out-of-network lab coverage. Billing questions regarding your lab bill must be directed to their respective billing departments.

There will be no charge if cancellations are made more than 24 hours before the appointment, and we will assist with rescheduling. Patients who fail to cancel their appointment within the 24 hours window will be charged a $35 late cancellation fee. If the patient fails to show for the appointment and no notice of cancellation is given, the patient will be charged a $50 cancellation fee. 

As of 12/01/15, we are no longer accepting new Medicaid patients. If you still have these services and would like to be seen at our office, you may fill out our Financial Hardship application here and send to our office in person or electronically to be considered as a discounted Cash Pay Patient on our affordable sliding scale.

 

Medicare Update (7/29/16): We have officially been approved as a Washington Medicare Provider. If you have been waiting to see your physician please call us at 425-369-1342 and select option 0 to schedule your appointment.

If you have been seen in the clinic for at least one year, you will need to set up an appointment in order to receive further refills of your medication. Request an Appointment
